APPLICATION FORM FOR THE PERSON CONCERNED
Article 11 of the Law on the Protection of Personal Data No. 6698 (“KVKK”) stipulates that the data subject may exercise the rights set out below by submitting a request to the data controller. The procedures and principles governing applications made by natural persons whose personal data is processed (“Data Subjects”) to the data controller are regulated by the “Communication on the Procedures and Principles for Applications to the Data Controller”. This form has been drawn up to enable us to fully identify your legal relationship with our company and, where applicable, the personal data processed by us, so that we may respond to your enquiry accurately and within the statutory timeframe. Should the information provided in relation to your requests via this Application Form be incorrect or out of date, or should an unauthorised application be made, our company accepts no liability for any claims arising from such incorrect information or unauthorised applications. In the event of submission by a representative, the original or a certified copy of the power of attorney, or a certificate of representation or authorisation, must be provided. Provided you submit your application in accordance with the specified procedures and principles, your requests will be processed free of charge as soon as possible and within 30 days at the latest, depending on their nature. However, should the process incur additional costs, a fee in accordance with the tariff set by the Personal Data Protection Board may be charged.
